The AZ Medical Marijuana Card Process

To be approved in Arizona for a medical marijuana card, a patient must have a qualifying medical condition that is recognized by the state of Arizona.

Patient Evaluation

The patient will be seen by our qualified staff and physicians for evaluation of their medical condition. This may involve review of outside records, consultation and physical exam.

Physician Certification

Once patient medical records have been established or reviewed and meet the state requirements for medical marijuana certification, the patient will then be able to be certified for the use of medical marijuana. During this visit patients will be able to have their questions answered before completing the visit.

Certification Processing

After the patient finishes with the certification visit the friendly front office staff will then guide the patient to help them with the rest of the process including taking a photo for the patient’s medical card, having the patient sign off on all necessary documents and finally checking out.

Receiving your Card

The certification card will arrive in the mail. Cards can take up to two weeks to be processed and mailed to the patient’s residence or mailing address (if requested). Our helpful staff will complete the application the same day patient’s come in for their office visit!
